Payments & Insurances AcceptedDavid is an in-network provider with most insurance plans, including HMSA, HMSA Quest, HMAA, UHA, AlohaCare, United Healthcare, Cigna, Anthem, Blue Cross/Blue Shield, ACI, Anthem EAP, Cigna EAP, Optum Health, and others. When using insurance, there is often a co-pay per session. David can assist you in determining your co-pay.
If you do not have insurance or would prefer to make private payments, individual sessions are $150 per 50-minute session. Sliding scale fees are accepted in some cases. Payments and co-pays are accepted by cash, check, or credit card, Venmo, or Square, are due at the end of session, and receipts are provided. Mahalo! |
